Is Advanced Primary Care Management the same as Chronic Care Management?
They are related but not the same. Chronic Care Management focuses specifically on patients with multiple qualifying chronic conditions. APCM is a broader primary care framework that combines care planning, coordination, medication management, transitional support, and enhanced communication. Our team can review your needs and explain which may be appropriate.
Does Medicare cover Advanced Primary Care Management?
Medicare Part B covers APCM for eligible patients when program requirements are met, and it is provided as an ongoing monthly service. Will I still see my primary care provider?
