Yearly visits
Annual Wellness Visit
A named Medicare service delivered monthly on top of your visits
Annual Wellness Visit
You have Medicare Part B and want a yearly prevention planning visit. Medicare describes it as a conversation to build a prevention plan, and says plainly that it is not a routine physical exam.
Annual Physical Exam
You want a hands-on, head-to-toe yearly exam. Medicare does not cover a routine physical exam, so coverage depends on your plan.
General Check Up

Do you offer annual wellness visits?
Yes. Annual wellness visits are a core part of our preventive care and include a review of your health, screenings, medications, and future care planning.
What is the difference between an annual wellness visit and an annual physical?
Medicare describes the yearly wellness visit as a conversation with your provider to build a prevention plan, and states that it is not a routine physical exam. An annual physical is a hands-on, head-to-toe examination. Medicare does not cover a routine physical exam, so coverage depends on your plan. Call us and we will help you book the right one.
Can I schedule a telehealth visit?
Yes. Telehealth visits are available for many follow-ups, medication questions, and chronic condition check-ins.

Chronic Care Management (CCM) provides ongoing, coordinated support for patients living with long-term conditions like diabetes, hypertension, or heart disease, between office visits.
What is Remote Patient Monitoring?
Remote Patient Monitoring (RPM) uses connected devices so your care team can review certain health readings from home when appropriate, helping catch issues sooner.
Do you help with referrals and care coordination?
Yes. Our team helps coordinate specialist referrals, labs, imaging, records, and follow-up steps so your care stays organized.
Do you offer work, school, and sports physicals?
Yes. We provide physical exams for employment, school, and sports participation, as well as general health clearance.
Do you help manage medications?
Yes. We provide medication reviews, refill support, and reconciliation to help patients take their medications safely and correctly.

PrimaryCareIM works with major insurance providers, including Medicare, BlueCross BlueShield, Cigna, UnitedHealthcare Sunshine Health, Ambetter and Aetna.
Coverage varies by plan, so please call the office where you plan to be seen and we will check your coverage before your visit.
Do you accept new patients?
Yes, PrimaryCareIM is currently accepting new adult and senior patients at both our Fort Myers and Lehigh Acres offices.
Which locations can I visit?
We have two Southwest Florida offices: Fort Myers, at 13670 Metropolis Ave #104, and Lehigh Acres, at 1150 Lee Blvd. #4.
Both are open Monday through Thursday, 8:00 AM to 5:00 PM. We serve patients throughout Fort Myers, Lehigh Acres, Cape Coral, and surrounding communities.
