Your health needs do not always fit neatly into one office visit. Advanced Primary Care Management (APCM) gives eligible patients more continuous support from their primary care team, helping keep care plans, medications, referrals, follow-up, and changing health needs better connected over time.

APCM is a Medicare Part B service, provided as an ongoing monthly service for patients who are eligible and who agree to take part.

What Is Advanced Primary Care Management?

Advanced Primary Care Management is a patient-focused approach designed to make primary care more continuous and coordinated.

Medicare describes it as a service in which your primary care provider takes extra steps to actively manage all of your health care needs.

Instead of relying only on individual office visits, your primary care team stays involved in organizing and managing your health needs over time. This may include maintaining a personalized care plan, coordinating with specialists, reviewing medications, supporting care transitions, and helping you stay on track with recommended follow-up.

For participating patients, the primary care provider and care team serve as the main point of ongoing care and help keep healthcare received from other providers and settings better connected.

A Medicare service, not a membership.

A chronic condition is followed over years. An acute problem (an infection, an injury, a sudden flare of something you already have) is handled now. Our care covers both, and the two are connected: an acute illness often lands on top of a chronic condition, and the plan has to account for that.

Round-the-Clock Access for Urgent Needs

Medicare requires practices providing APCM to give participating patients 24-hour access to a member of the care team who can see their medical information, for urgent needs. Participating patients also have expanded ways to reach the care team between visits, such as secure messaging, when clinically appropriate.

Not every part of this applies to every patient every month. Your care team provides what your situation calls for.

APCM does not replace regular primary care. It extends and organizes the care relationship between visits.

Advanced Primary Care Management vs Chronic Care Management

APCM and Chronic Care Management (CCM) are related care-management services, but they are not the same. The right option depends on your healthcare needs and eligibility.

Chronic Care Management

CCM is designed specifically around ongoing support for patients with multiple qualifying chronic conditions.
Medicare says you must have two or more serious chronic conditions that you expect to last at least a year in order to qualify, and that your provider will ask you to sign an agreement to receive the service on a monthly basis.

Advanced Primary Care Management

APCM is a broader primary care framework that combines care management, comprehensive care planning, care coordination, transitional support, medication management, and enhanced communication into one monthly service.
Medicare includes a level of APCM for patients with one chronic condition or none, so it is not limited to people managing several conditions.

Which one applies to me?

You do not need to decide which program is right for you on your own.

Your PrimaryCareIM care team can review your needs and explain which form of care management may be appropriate. Medicare will only pay one provider for APCM in a given month, so if another practice already provides a monthly care management service for you, say so — it affects what is possible.

Start With Your Primary Care Team

Your first visit covers your history, every condition you are managing, every medicine and supplement you take, your past test results, and which specialists you already see. If your care started somewhere else, we work from those records rather than starting you over.

Agree to Take Part

Nothing starts until you say yes.​ Medicare requires your consent, in writing or spoken out loud, and it is recorded in your chart. You will be told that only one provider can be paid for this service in a given month, that cost sharing may apply, and that you can stop at any time.

Coverage & Participation

Does Medicare Cover Advanced Primary Care Management?

Yes. Medicare Part B covers Advanced Primary Care Management for eligible patients when program requirements are met, and it is provided as an ongoing monthly service.

Who is eligible

You need Medicare Part B, and one primary care provider has to be acting as the main point of contact for all of your health care needs.

Medicare sets up the service at three levels, based on your health: one level for patients with one chronic condition or none, one for patients with two or more chronic conditions, and one for patients with two or more chronic conditions who also take part in the Qualified Medicare Beneficiary program. You do not need to work out which level applies to you. Your provider does that.

What you pay

Medicare states that after you meet the Part B deductible, you pay 20% of the Medicare-approved amount for this service.​

Because APCM is a monthly service, that amount can appear each month you receive it.

Many people who have both Medicare and Medicaid pay nothing for these services.

If you have a Medicare Advantage plan or a secondary insurance, your costs work differently. Medicare advises contacting your plan to find out what you would pay.

What you are agreeing to

Participation requires patient consent, and patients may stop participating.

Medicare requires that your consent (spoken or written) is recorded in your medical record, and that before you agree you are told three things:

  • Only one provider can be paid for this service in a given calendar month.
  • Cost sharing may apply.
  • You can stop receiving the service at any time.

If you have not been seen at our practice in the last three years, Medicare requires a regular office visit before the service can begin.
